Multiple myeloma (MM) is characterized by clonal proliferation of plasma cells in the bone marrow. Extramedullary plasmacytomas (EMP) can occur either at initial diagnosis of MM or later during the course of the disease. EMP can affect any area of tissue, but the pleura, lymph nodes, soft tissue, liver, skin, lungs, central nervous system, genitourinary system, breast and pancreas are more frequently involved. The presence of EMP in MM is associated with aggressive disease leading to shorter overall survival and progression-free survival [1]. Multifocal EMP represents an even rarer manifestation in the course of MM (1% of all plasma cell malignancies) [2], [3] and cases of multifocal visceral relapse after transplantation are exceptional [3]. We describe here a rare case of a patient with IgG/k myeloma, who relapsed soon after autologous stem cell transplantation (ASCT) with a massive multifocal visceral extramedullary plasmacytoma. There was no evidence of myeloma marrow involvement nor monoclonal serum protein.
