This article describes some of the reconstructive potential offered by the myocutaneous latissimus dorsi flap after excision of various types of lesion in the chest and root of the upper extremity. The myocutaneous latissimus dorsi flap confirms its efficacy in soft tissue reconstruction in these regions. Anatomic interruption of the thoracodorsal trunks does not prevent the flap being raised since it is adequately drained by integral thoracic and dorsal branches. This inverted-flow vascular pedicle does not affect flap size and, despite its relative shortness, permits coverage of areas such as the apex or anterior part of the shoulder without excessive tissue impairment or strain.
