From chronic health center to territorial operations center: the experience of the ASL Tuscany South East

The ASL TSE was founded in 2016 from the union of 3 ASLs: Siena, Arezzo and Grosseto. The population is older than the regional one: easy access to care must be guaranteed despite the wide dispersion across the territory. Therefore, the Chronic Health Center takes on a crucial role. The study aims to describe the role and activity data of the Center in the ASLTSE and its evolution to COT. The activity data and indicators of the Center were retrieved from the company platform "Datafinder-ACOT". Performance's analysis was carried out by using the 2022 data from the MeS of Sant'Anna of Pisa. In 2023, the number of reports to the Center was 10,094, a significant percentage in home care and intermediate care. The performance data shows that the ASL TSE has a hospitalization rate lower than the regional average, proving the effectiveness of the territorial health network guaranteed by patients' management by the Center.
