Cardiovascular diseases are still the main cause of death among women despite the improvements in treatment and prognosis achieved in the last 30 years of research. The determinant factors and causes have not been completely identified but the role of "gender"is now recognized. It is well known that women tend to develop cardiovascular disease at an older age than men, and have a high probability of manifesting atypical symptoms not often recognized. Other factors may also co-exist in women, which may favor the onset of specific cardiac diseases such as those with a sex-specific etiology (differential effects of estrogens, pregnancy pathologies, etc.) and those with a different gender expression of specific and prevalent risk factors, inflammatory and autoimmune diseases and cancer. Whether the gender differences observed in cardiovascular outcomes are influenced by real biological differences remains a matter of debate. This ANMCO position paper aims at providing the state of the research on this topic, with particular attention to the diagnostic aspects and to care organization.

Le malattie cardiovascolari rimangono la principale causa di mortalità tra le donne nonostante il miglioramento delle cure e della prognosi negli ultimi 30 anni di ricerca. Le cause e i fattori determinanti non sono ancora completamente definiti ma è ormai riconosciuto il ruolo del “genere”. Sappiamo che le donne tendono a presentare una malattia cardiovascolare in età più avanzata rispetto agli uomini, con alta probabilità di manifestare sintomi atipici spesso non riconosciuti. Inoltre nella donna possono coesistere altri fattori che possono facilitare l’insorgenza di specifiche malattie cardiache ovvero quelle ad eziologia sesso-specifica (influenza degli estrogeni, patologie in gravidanza, ecc.) e quelle a diversa espressione di genere (fattori di rischio specifici e prevalenti, patologie infiammatorie ed autoimmuni, patologia oncologica). Resta poi ancora dibattuta la questione relativa alle differenze di genere osservate negli esiti cardiovascolari ovvero se siano influenzati da reali differenze biologiche. Questo position paper ANMCO si pone l’obiettivo di presentare alla comunità cardiologica lo stato dell’arte della ricerca sull’argomento con particolare attenzione agli aspetti di diagnosi e organizzazione della cura.
