Signet ring cell carcinoma (SRCc) of the esophagogastric junction (EGJ) is a histological subtype of adenocarcinoma (ADC). Its incidence in the literature series is variable, mainly due to a lack of reporting standardization and to variable incidence in different populations. Due to the infiltrative nature and its aggressive behavior, many efforts were made to understand its complex biology and to set up an effective preoperative workup, trying to forecast the tumor responsiveness in order to avoid toxicity of chemotherapies. Surgery, chemoradiation, and hyperthermic intraperitoneal chemotherapy (HIPEC) are only some tools that can be used in the multimodal tailored approach to this aggressive tumor.
