Various complications have been reported with the use of local anesthetics, including local tissue necrosis. The authors present a case of this complication with the presence of necrotic ulcers of the palate. A female patient who received anesthesia with articaine 1:100,000 developed necrosis of the palatal mucosa. The authors stress the importance of surgical intervention at the site of the necrotic mucosa. The treatment of oral mucosal ulcers requires great attention. The main objective is to achieve complete tissue healing as rapidly as possible. During surgical treatment of necrosis related to local anesthetics, it is always advisable to use a different anesthetic agent and one that does not contain adrenaline. The absence of the vasoconstrictor increases bleeding during surgery but improves blood flow to the wound. One month after surgery, complete healing had been achieved. Appropriate systemic and local drug therapies are essential to successful treatment and tissue healing.
