Nursing home care is defined as the healthcare and social interventions for particularly fragile patients directly in their homes, with the aim to stabilize their clinical condition, to limit the functional decline and to improving the quality of daily life. In recent years the need to monitor these interventions has emerged, in order to identify the most appropriate tools for assessing the quality of them. Description of the problem In 2016 Tuscany Region started a project involving the Territorial Nursing Unit of Valdinievole (Pistoia) to monitor the nursing home care. A special check-list was created and 3 key points were examined: the acquisition of the consent form for patients; the measurement and management of pain through a validated tool (ABBEY Scale); the evaluation of nurses’ work assessing the general health status of patient through the ASGO scale, normally used for hospitalized patients. Results In August-September 2016, we analyzed “at random” 91 of 993 nursing boards of patients who were treated by nursing home care. The only inclusion criterion was to have benefited from it at least 28 times, until 31/7/2016. The indicators were verified by a group composed by different professionals. Lessons At the end of the analysis of nursing boards some critical points emerged and it was decided: to improve the consent form, that should be kept in the board even after storage; to program meetings to better manage patients’ pain to use a computerized ASGO to avoid mistakes to adopt a unique and easy language to compile the nursing board, and to create a register with the signs of operators. Key messages: The work represents a pilot project to detect a useful tool to verify the quality of nursing home care. Our work evidenced some critical issues of the system that we are trying to overcome and that we want to monitor afterwards.
